Not sure about the gen-2 stuff, but the "classic" way was to first sculpt the figure by hand, then carve the steel mold using a 3D pantograph milling machine.

Not sure if it's done much in the military model industry, but a lot of the sci-fi wargaming figures are initially sculpted in a much larger scale. The pantograph then does the re-scaling during the milling progress.

As to the gen-2 stuff, I'm not sure if the initial model work is done via a master and then laser scanned into the computer for CNC milling of the mold, or if the whole figure is designed in a CAD program. But I'm almost positive the mold is carved via a CNC mill for the figures. Doesn't make any sense to CAD/CNC the mold for the vehicles, then pantograph the figures.
