It's been noted above, but the primary difference is the turrets. The "KV Big Turret" kit includes the MT-1 turret which was shorter (length-wise), higher, had a different mantlet and different angles than the production MT-2 turret included in the KV-II kit.
From the evidence I've been able to gather, the MT-1 turret was apparently only fitted to prototype KV hulls, although the turrets were swapped about a bit so the same hull may have had "big" and "small" (76.2mm) turrets at different times.
Different sources give the number of MT-1 turrets as between 4 and 6, so it was a rare beast. The remainder of the 334 KV-II's were fitted with the MT-2 turret. However, vehicles with the MT-1 turret saw service in the Russo-Finnish Winter War of 1939 and in the early stages of Barbarossa in 1941, mostly in the northern regions.
As an interesting aside, the Trumpeter "KV Big Turret" kit is the only kit in plastic that provides the very early pattern return rollers with rubber tires and six reinforcing ribs. These are useful if you're building a KV-1 Model 1939.
