I've examined most of the Tiger 1 at Bovington when it was dissassembled. EVERY SINGLE BOLT had some sort of precaution against working loose; usually this was a flat metal washer threaded under the bolt head, folded up at one point to engage the side of the bolt head, and folded down somewhere else to engage the metal. Sometimes a hole would be drilled specifically for these washers to engage.
If they didn't want to drill holes in the underlying steel, and the bolt were large enough, and there were multiple bolts close together, they had the option of drilling a hole in the bolt head and running a wire through each pair of bolts. But they used many other tricks too.
