The 76mm gun began to appear in July 1944. Its introduction was gradual, due to resistence from commanders and (initially) from the US tankers themselves, and 76mm-armed tanks seem to have been allocated on a farily ad hoce basis. In October 1944 2nd Armoured had 156 75mm-armed M4's and 68 76mm-armed M4's, while 7th Armoured had 94 and 12 respectively. By January 1945 the proportions were 117/21 and 61/36 respectively. These statistics do not differentiate between M4 types because the US Army did not record this information, but the M4A3 eventually made up 44% of the M4's in the US Army (this info comes from Zaloga's 'US Armoured Divisions').
The M4A3 began to appear in US armoured divisions after the commencement of the Normandy campaign; up until that point they were using M4's and M4A1's. There was no official policy on allocation of the new M4A3's; some formations had almost exclusively M4A3's (like 9th Armoured), while others had a mixture of types as the M4A3's replaced losses.
Apparently, it was uncommon to mix M4A3's with the other types at unit level because they used different engines, and it complicated the logistics. You will see many photos of M4's and M4A1's in the same unit (including different versions of both types) but you will see few photos of M4A3's alongside M4/M4A1's.
